Illinois state law (called Direct Access)
allows physical therapy evaluation and treatment as long
as there is a relevant
diagnosis made by a physician within the past 12 months.
Insurance companies may require a prescription from a physician
for every 30 days of care. Body Evolve does not submit
for payment directly to insurance carriers. However, we provide
patients with a detailed statement to do so themselves.
We
request that payment for physical therapy be made at the
time of service. If your insurance carrier requests additional
information for reimbursement, we will be happy to assist
you.

Cash, personal checks, Visa, and MasterCard
are accepted. Appointments cancelled with less than
24 hours
notice will be charged for a full session.
